Appointment Cancellation

We understand that situations arise, but in order to run an efficient medical practice and offer every patient the highest quality of medical care, we have found that it is necessary that patients give us at least 24 hours notice to cancel an appointment. This will allow us to offer your appointment to another person who may need it. We do reserve the right to bill you $50 for each no show or late cancellation. This fee is not covered by insurance and will be your responsibility to pay. While we make every effort to be accommodating and considerate of our patients, we appreciate your understanding and consideration for our appointment cancellation policy.

Billing and Insurance

As a courtesy to our patients, we will attempt to obtain the most accurate insurance information about your plan prior to appointment day. However, this information may sometimes not reach us in time or may not be properly communicated to us. While we make every attempt to be of assistance to you as far as insurance matters are concerned, please understand that it is ultimately the patient’s responsibility to be knowledgeable of coverage limits of the policy with regards to physicians seen or services provided.

Please be aware that any copays, coinsurance and/or deductibles are the responsibility of the patient. Since it is our policy to collect these amounts at the time of your visit, please arrive at your appointment prepared. For your convenience, we accept cash, checks, credit cards, and debit cards.

 


Telephone Calls

IN THE EVENT OF AN EMERGENCY, PLEASE DO NOT CALL OUR OFFICE. CALL 911 IMMEDIATELY OR GO TO THE NEAREST EMERGENCY ROOM.

For non-urgent questions or issues during office hours, your calls will be answered by our staff. If the person that you need to reach is unavailable when you call, please simply leave a message and your call will be returned by the appropriate member of our staff. We kindly ask that you not leave multiple messages, as this will only delay a response and possibly cause confusion.
After office hours, your calls to our office are routed to the Physician on call through our Answering Service.

Prescription Refills

Need a prescription refilled? Please contact your pharmacy at least three business days in advance so that the pharmacy may forward a request to our office. Please understand that some prescription refills may require a follow-up office visit. Please allow 48 hours from the time we receive the refill request from your pharmacy. To confirm that a refill has been completed and is ready for pick-up, please contact your pharmacy directly.

Medical Records

We understand that you may need for us to release your medical records to another provider. In this event, please simply let us know and we will provide you with a Medical Records Release form to complete. When completing this form, you will need to inform us of the name, address, phone and fax number of the physician and/or office in which the medical records will be sent. We make every attempt to send medical records in a timely manner, but we do ask for you to allow up to 10 business days for us to complete. Fees may apply.
